This portal supports care requests, shared appointments, administrative reminders, and dated medical-history and appointment notes recorded by authorized care staff for About Your Care Home.

Information you share

Your account includes your name, email address, and any phone number you provide. Care requests include the care recipient’s name, address, and your message. Please share only the information needed for an initial conversation; do not include medical records or identification documents in a request.

Who can see it

Your request is available to authorized administrators. Shared profile details and appointments are available to authorized family accounts and team members according to their permissions. The separate Medical history & appointment notes section is restricted to authorized staff and is not visible to Customer or family accounts. Linking a staff note to an appointment does not share the note with families. Internal administrative notes also remain separate from information shown to families.

Images and letters

Authorized staff with editing permission can attach JPEG, PNG, or WebP images up to 10 MB to a care-team note. These originals are stored privately in Supabase and can be viewed only by authorized staff. Customer and family accounts cannot upload or view them, and they are not included in reminder emails. Removing an image from view retains the original file.

Downloaded PNG copies add a footer below the complete image reading “Downloaded from AboutYourCareHome.com”. Large copies may be resized to fit a 12-megapixel image area before the footer is added. Previewing or downloading an image does not change the stored original.

Appointments and optional reminders

You choose where reminders are sent, then turn them on for the calendars you want to follow. Messages contain a brief notice and a link to sign in, rather than detailed care information. We use Supabase for accounts and portal data, Cloudflare for the website and support-email forwarding, Resend for outgoing email. These providers process the information needed to perform those services.

Changes and history

Corrections to entries in Medical history & appointment notes retain earlier versions for authorized staff review.

Account information and requests

You can update your contact details in your account. For corrections, access questions, or information that should no longer be shared, email ContactUs@AboutYourCareHome.com. Please do not email passwords, verification codes, or private care information.

Archiving a care profile or removing account access retains associated care records, earlier note versions, image attachments, and delivery history. Contact the care-home team to discuss corrections, access, or retention requests.
